How to refer
Get in touch
Call or email our referrals team to make an initial enquiry. We'll discuss your client's needs and confirm whether we can help.
Send referral information
We'll ask for relevant background information including care history, current needs, risk assessments, and any existing support plans.
Assessment
Our clinical and operational leads will conduct a thorough assessment — meeting the individual where possible — to determine the right level and type of support.
Placement & transition
Once agreed, we'll work with you to plan a safe, supported transition into the placement, with regular communication throughout.
