Our Services

Specialist Supported Living Services

We provide high-quality, person-centred supported living for adults with complex needs — designed around each individual's goals, preferences, and clinical requirements.

What is supported living?

Supported living enables adults with complex needs to live in their own home or a shared setting, with the right level of support to maintain independence and quality of life. Unlike residential care, supported living separates housing from support — giving individuals greater choice, control, and autonomy over their daily lives. Swancare Solutions provides the support element, working closely with housing providers, commissioners, and clinical teams to create safe, stable, and fulfilling environments.

High-Support Supported Living

For individuals with the most complex needs, we provide intensive, 24/7 support in purpose-adapted settings. Our teams are trained to manage challenging behaviour, risk, and complex clinical presentations — always within a positive, therapeutic framework.

  • 24/7 waking night support
  • Positive Behaviour Support (PBS)
  • Risk management planning
  • Clinical liaison and oversight

Step-Down & Transition Support

We work with NHS inpatient units, secure services, and residential care providers to support safe, planned transitions into community-based supported living. Our step-down pathway is designed to reduce hospital admissions and support long-term community integration.

  • Hospital discharge planning
  • Gradual transition programmes
  • Joint working with clinical teams
  • Relapse prevention planning

Community Integration Support

We support individuals to build meaningful lives in their communities — developing social connections, accessing education and employment, and participating in activities that matter to them. Our support workers act as enablers, not gatekeepers.

  • Community access and activities
  • Education and employment support
  • Social skills development
  • Independent living skills

Floating Support

For individuals who require lower levels of support, our floating support service provides flexible, time-limited assistance to help people maintain their tenancy, manage daily living, and access community resources.

  • Tenancy sustainment
  • Benefits and finance support
  • Health and wellbeing guidance
  • Signposting to community services

Crisis Prevention & Management

Our teams are trained in de-escalation, crisis intervention, and proactive risk management. We work with individuals and their clinical teams to develop robust crisis plans that reduce the need for hospital admission.

  • Crisis planning and prevention
  • De-escalation training
  • On-call management support
  • Emergency response protocols

Bespoke Packages of Care

Every individual is different. Where standard service models don't fit, we work with commissioners and clinical teams to design bespoke packages of support tailored to the specific needs, risks, and aspirations of the individual.

  • Individually designed support plans
  • Flexible staffing models
  • Multi-agency collaboration
  • Regular review and outcome reporting
